We are beginning work on our 2011-2012 church directory and need your information. If you have changed phone numbers, addresses, email addresses, or any other pertinent information, please indicate that on the form here. Unless they are your primary means of contact, work and cell numbers will not be published in the directory but used only by the church staff. Also, if you have addresses for your college student(s), please complete that information on the form as well.
